2009-2010 GRANITE STATE LEAGUE

TEAM CREDENTIALS BOOK REQUIREMENTS

Below is a list of documents that need to be assembled in a book (Credentials Books) and needs to be with the coach at all practices and games. This book is a requirement of the Granite State League.

All Consent to Treat forms should be completed as soon as possible and coaches should have on hand in case a player gets hurt and a parent is not available. All player forms should be in alphabetical order and all game sheets should be in date order. You will be receiving a Credential Sheet from me before the end of the season. Please place this in the front of your book for the State Registrar to use when certifying your credentials. Do not mark up this sheet. I will also be sending you your signed rosters to include in your Credentials Book.

Waiver of Liability

- USA Hockey, Coaches Certificate of Education Card (Copy both sides)

- Participant Town Birth Certificate (Hospital Certificate or Passport not acceptable)

- Copy of all Granite State Game Sheets (Date order)
